Health Home Care Coordination for Adults

Program Overview

The goal of the Health Home Care coordination program is to provide access to all the services an individual needs to stay healthy and out of the emergency room and spend less time in hospitals. The care coordinator ensures the individual is getting regular care and services from doctors and providers, finding a safe place to live, and finding a way to get to medical appointments. Care Coordinators build linkages to other community and social supports with the main focus on the needs of people with multiple chronic conditions.

Qualifications

Two chronic conditions and Medicaid eligible.

The Reason

The program provides case management services to adults with serious behavioral and/or physical issues.

The Solution

Provide monthly in person sessions, able to accompany member to medical appointments and provide interpretation and assist with paper work.

The Implementation

Intake – Potential members can call program manager and or email requesting initial interview for intake. Upon intake appointment outreach coordinator will determine if potential candidate qualifies for program.
