The Hispanic Counseling Center (HCC), Scattered-Site Supportive Housing Program (SHP), supports mentally ill adult clients in the challenging task of finding appropriate housing opportunities and assists in securing permanent housing.

The Reason

The Supportive Housing Program helps decrease the disparity of the low income mental health adults with difficulty assessing and retaining safe, secure affordable housing.

The Solution

The Supportive Housing Program helps clients who are ready to live independently by providing them with monthly rent stipend and support and referral services. This ensures that clients can remain housed and thrive in the community.

The Implementation

The Supportive Housing Program provides clients with the opportunity to live independently and enjoy the same rights as any other tenant in the community. The program offers apartments scattered throughout Nassau County and provides person-centered services that are tailored to the client’s needs. Supportive Housing Staff meets with clients one or more times per month to coordinate services.
